Affidavit Of Good Behaviour & Parents/Guardians Undertaking Form Required To Complete Clearance
Sequel to the internal memorandum released by the Directorate of Students’ Affairs on Friday, 10th May 2019, All Studentss are requested to go to their various faculties to collect an affidavit of Good behavior and Parents/Guardians Undertaking forms and have their Parents/Guardians sign on it as it is required to complete final clearance at the faculty.

COLLECTION OF AFFIDAVIT OF GOOD BEHAVIOUR AND PARENTS/GUARDIANS UNDERTAKING FORM
All Students are to go to their various faculties to collect an affidavit of Good behavior and Parents/Guardians Undertaking forms.
You are also to note that necessary information must be filled on all portion provided on the forms and must be duly signed by your Parents/Guardians as this will enable you to complete your final clearance at the faculty.
In view of this, you are to submit the duly signed forms with the photocopy of Two thousand Naira only (#2,000.00) receipt fee paid for damages to get a clearance form from your faculty .
